A reformatted version of an IRS form, such as
Form 1099-B for stock sales,
Form 3921 for exercises of incentive stock options, and
Form 3922 for purchases in qualified employee stock purchase plans. To issue the information reported on these and some other IRS forms, an employer or financial institution may use its own reformatted statement instead of the official IRS form. The format must follow guidance in
IRS Publication 1179 to present the same information and use the same terms as those the required IRS form. The purpose of the different layout and design is to make the information perhaps easier for recipients to understand and use.
